Complete Your Forms
Arrive 10 minutes early to complete your new patient forms, or ask us to email them in advance so you can fill them out at home. We'll need your health history and insurance information.

What should I bring to my first appointment?
Please bring a valid photo ID, your insurance card (if applicable), and a list of any current medications. If you have recent dental X-rays from a previous dentist, we'd love to have them — just ask your former provider to send them over.
Do you accept new patients without a referral?
Absolutely. We welcome new patients of all ages without a referral. Simply call us or book online and we'll get you scheduled as soon as possible.
How long is the first appointment?
A comprehensive new patient exam typically takes 60–90 minutes. This gives us time to do a thorough assessment, take necessary X-rays, and have a meaningful conversation about your oral health — without feeling rushed.
Do you offer direct insurance billing?
Yes. We direct-bill most major insurance providers so you only pay the remaining balance at the time of your visit. Our front desk team can verify your coverage before your appointment.
What if I'm nervous about the dentist?
You're not alone — dental anxiety is very common. Please let us know when you book so we can take extra care to make you comfortable. We also offer nitrous oxide and oral sedation options for patients who need additional support.
